The fourth Death Star was built in tandum with its sister, the Death Star V in order to destroy the New Republic and reestablish the Empire.

The Death Star IV and V Project began as an idea of fusing the two Superlasers of individual Death Stars into a much more potent Megalaser. Grand Hoff Chebin, looking to solidify his standing in the Imperial Remnant after The Emperor's apparent death, took the reins of the project and began overseeing the construction of the two stations. The seperate Death Stars were built simultaneously, but in separate systems in order to better hide and protect the secret project. It would take three years, but the dream was almost realized, with the Death Star IV completing construction a month ahead of schedule.

The New Republic caught wind of the new project, however, and Red Squadron was assembled to destroy the superweapon. They realized that the Death Star IV had the same design flaw as the Death Star I, a two-meter wide exhaust port. The Republic's X-wing fighters were more of a match for the Empire's aging TIE Fighter fleet, but the Imperials still managed to inflict heavy casualties on the Rebel pilots. It just so happened, however, a lost Star Tours transport entered the fray. Armed with its own laser cannons and proton-torpedo launchers, the small Star Tours Starspeeder 9000, piloted by the malfunctioning droid pilot "Captain Rex," led a trench run against the battle station and landed two proton torpedoes in the thermal exhaust port, destroying the newly completed Death Star and ensuring victory for the New Republic fleet. Construction was canceled on the fifth Death Star with Grand Hoff Chebin, the project leader, dying in the ensuing explosion.

The current whereabouts of Captain Rex are unknown, but his passengers survived the ordeal with only minor bruises and scratches.
